What Is a Heat Rash and Why Does It Happen on Tattoos?
A heat rash (miliaria) forms when sweat glands become blocked and sweat gets trapped under the skin. The result is a cluster of small, itchy, prickly bumps. In some cases these bumps are red (miliaria rubra), in others they are clear, fluid-filled blisters (miliaria crystallina). Both are heat rash.
Healing tattoos are more susceptible than ordinary skin for several reasons. The area is already inflamed and compromised as a wound. Heavy aftercare products or tight clothing create a layer over the skin that traps heat and sweat. In summer months especially, the combination of heat, humidity, and a new tattoo creates ideal conditions for sweat glands to get blocked.

Normal tattoo healing looks like redness and swelling in the first 24 to 48 hours, then a gradual decrease. From days 5 to 14 you get flaking and peeling. Itching is common. There are no clusters of bumps.

Remove the source. Get somewhere cooler and air-conditioned. Remove tight clothing over the tattoo. If you have a bandage on the tattoo that has been on more than 24 hours, remove it — you may be trapping sweat under the film.
Step 2: Wash the area gently
Clean the tattoo with cool or lukewarm water and a gentle fragrance-free cleanser. Wash away sweat and any heavy product residue that may be clogging the glands. Rinse thoroughly. Pat completely dry with a clean paper towel. Do not rub.

Wear the loosest, most breathable clothing possible over the area. Natural fabrics like cotton are better than synthetics. If the tattoo is somewhere you can leave exposed to air safely, do so. The rash needs airflow to resolve. With this routine, most heat rashes clear up within one to two days.
How to Prevent It
The main prevention strategy is not letting heat and sweat get trapped. In practice: do not apply thick, occlusive products like petroleum jelly during hot weather; avoid tight, non-breathable clothing over the tattoo; if using a second-skin bandage in summer heat, check the seal regularly and remove at the first sign of a rash at the edges; stay out of direct sun and avoid heavy sweating in the first week.
